We’re looking for someone to join our team as a Director, Home Care Services (Hospice): Provides leadership and direction to all home care site directors.

Collaborates with leadership to develop the hospice strategy, vision and goals and align it with the system strategy and initiatives.

Drives, operationalizes and implements initiatives.

Works directly with each agency’s leadership/management.

Serves as the key contact for the acute setting and markets related to hospice initiatives.

Essential Functions: Provides oversight for hospice overall operational performance and expense management and identifies strategies towards efficiency and efficacy while maintaining service quality and patient safety Ensures processes or work flows are best practice and are standardized and maintained Develops action plan for system hospice quality goals and walk to top decile performance Provides leadership and support for hospice medical directors and be responsible for providing or arranging for up to date education and training Ensures hospice team has adequate and continued education and training so they are up to date on the latest information as it relates to hospice care Integral part of the organization’s leadership team in planning and executing implementation of any new initiatives, also keeping in mind system initiatives You’ll be successful with the following qualifications: Education: Bachelor’s Degree in business and/or nursing: Required Master’s Degree: Preferred Work Experience: Seven years experience in hospice management: Preferred Five years experience in home care leadership: Preferred Relevant hospital experience: Preferred Required Licenses/Certifications: Valid Driver’s License (DL): Required
