Our Rehab and Nursing Centers have been providing services in West Michigan for 20+ years. Our residents range in age, diversity and acuity levels. At our Kalamazoo Ave. facility, we help residents live a healthy and active lifestyle to the greatest and safest extent possible.

Key/Unique Facility Details:

  • Our two-floor facility was constructed in 2016, replacing the previous building. It has a 76 bed Post-Acute Rehabilitation unit and an 88 bed Long Term Unit.
  • Each floor has designated units that focus on patient specialties such as: General rehab, neuro rehab, traumatic brain injuries, cancer, and other specialties.
  • Recreational therapy (group and one on one), animal therapy, and garden spaces provide residents with a variety of day to day activities to keep them engaged.
  • Physical activities, games, music groups, movies and organized outings (i.e. sporting events, zoos, store, etc) are available to residents.
  • Beauty salon and barber services are also available.

Our typical shifts are as follows:

  • 12 hours shifts days: 6:30 a.m. to 7 p.m., Every third weekend
  • 12 hours shifts nights: 6:30 p.m. to 7 a.m., Every third weekend
  • 8 hours shift:  6:30 a.m. to 3 p.m. or 2:30 p.m. to 11 p.m., Every other weekend
  • Accommodations for different schedules may be made but not guaranteed

LPN Position Summary:

Under the direction of the RN / Mid-level Provider / Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care.

RN Position Summary:

Delivers nursing care responsibly and accountably as described professionally by the American Nurses Association (ANA) and legally by the State of Michigan. Demonstrates competency through the critical thinking model known as the nursing process. This includes assessment, diagnosis, outcomes identification, planning, implementation, and evaluation. Upholds the standards of professional performance as described by the ANA. This includes ethical practice, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice, professional practice evaluation, resource utilization and environmental health. Strives to achieve optimal outcomes. 

Qualifications

  • Associate’s Degree Nursing or LPN Certification – Required
  • RN or LPN license in State of Michigan – Required upon hire
  • Basic Life Support (BLS) required within 90 Days
