POSITION SUMMARY:
The purpose of this position is to provide skilled nursing care to patients requiring professional nursing service in accordance with physician’s orders and in compliance with state and federal regulations. Responsible for supervision of clinical staff in the provision of care to patients.
ESSENTIAL FUNCTIONS: To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation; including but not limited to:
- Evaluate the learning ability and knowledge of the patient and caregiver to determine what teaching will be required
- Evaluate the patient’s environment to determine what assistance will be available and what assistance will be needed
- Develop an individualized plan of care based on assessment findings, supporting documentation, and physician’s orders for each patient
- Supervise and evaluate the care provided by the certified nursing assistant as needed and at a minimum of every fourteen days
- Conduct monthly licensed practical nursing’s supervisory visits
- Make recommendations and demonstrate follow-up on assessment findings appropriately
- Complete OASIS assessments accurately according to company policies
- Incorporate new technologies and interventions for patient care
- Participate with the staff, patient/representative(s) and physician in discharge planning activities and completes the physical therapy discharge summary according to company policy
- Must be willing to travel to other office locations to assist with staffing if needed
- Participate in inter-disciplinary case conference and collaborate with members of the health care team to accomplish patient care goals
- Participate in rotating on call schedule
- Collaborate with physicians, patient/representative(s), nurses, therapists, social workers and other members of the care team to develop and implement a patient individualized plan of care/treatment plan
Required Skills:
- Work requires advanced knowledge of the Homecare Industry, Medicare Regulations, CMS, and other State/Federal regulatory changes
Required Work Experience:
- One (1) year experience as a Registered Nurse, preferred
- Acute care experience with the adult population in a medical-surgical environment
- Previous home health or hospice experience preferred
Licensing/Certification/Education Requirements:
- Graduate of an accredited school of Nursing (BSN Preferred)
- Current Registered Nursing License
- Current CPR Certification
Other Requirements:
- Position may require unscheduled overtime, week-end work
- Must be willing and able to work flexible hours
- Travel required
- Must have reliable transportation
Physical Requirements:
- Job will require frequent lifting of objects up to 40 pounds
