Nurse Specialist- Full Time/Salaried Home Health Nurse

Centria Healthcare is looking to hire caring, professional and dedicated LPNs & RNs to serve the pediatric clients in the Metro Detroit area. The ideal candidate will have experience working with the pediatric population that have tracheostomy, ventilator dependent and/or require enteral feedings within a home setting. Job Summary

As a Nursing Specialist at Centria Healthcare, you will provide home nursing care after hours, days, evenings and weekends. This is an excellent opportunity for a nurse who enjoys using multiple nursing skills and is passionate about providing the highest quality of care. You will be assigned to two shifts per week on a biweekly scheduling basis, you will also have two to three assigned on call days per week that you must be prepared to cover on call needs as well as last minute call offs for the organization. This position is designed to provide superior care to our pediatric clients suffering from respiratory complex conditions in the Metro Detroit area.

We are currently looking for 3 high level nurses. The openings are for (one) dayshift nurse and (two) nightshifts nurses.

Required Qualifications

, Skills, And Knowledge

  • Maintain Nursing License in good standing
  • One-two years’ experience in a healthcare setting; Home healthcare experience preferred
  • Experience with traumatic brain injury (TBI), spinal cord injury (SCI), tracheotomy, mechanical ventilation, Feeding tubes, pediatrics and adults
  • Excellent oral and written communication skills
  • Ability to follow verbal and written instructions
  • Ability to utilize computers/tablets and computer software; iPad/electronic charting
  • Ability to meet background check requirements, including, but not limited to no findings against the employee relating to client abuse or neglect, or misappropriation of client property
  • Must obtain CPR before beginning work. Certification and/or classes must be done in a live or hybrid course (not online course), and is at the cost of the employee.*
  • Must submit negative Tuberculosis (TB) test results.

Required

Availability

  • Must be available to work days/nights and weekends; rotating weekends
  • On-call schedule identified at the beginning of each month
  • Must maintain availability as identified at beginning of each month.
  • Available for administrative time to review care plan prior to each assignment and ask any needed questions/elicit support from clinical team at HQ

Responsibilities

/Essential Functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

  • Demonstrate efficient teamwork with the staff
  • Demonstrate organizational and time management skills
  • Support quality improvement practices
  • Perform nursing procedures according to agency policy and procedures
  • Work under the direction of DDOCS and RN Clinical Supervisors
  • Monitor reactions and client progress using observation, assessment and evaluation skills
  • Perform shift tasks, care plan and medication quality assurance and report out to Clinical Leader findings after each shift
  • Provide assistance with hygiene such as bathing, oral care, grooming and dressing
  • Provide assistance with light housekeeping and meal preparation
  • Perform and record accurate measurements (i.e. vital signs, or intake/output as instructed in the care plans)
  • Observe and report any safety hazards found in the client’s home or any significant observations regarding the client
  • Educate clients and family members according to disease control, medications, POC, treatment options and home care procedures
  • Report to physician and DOCS of adverse findings
  • Follow accreditation regulations
  • Coordinate and monitor client care and services
  • Comply with HIPAA regulations in and out of the office
  • Follow infection control policy in and out of the office
  • Document shift according to the accrediting guidelines
  • Maintain client records according to policy and procedures
  • Participate in in-services, workshops, seminars and self-study courses annually
